Addressing gaps in social determinants of health (SDoH) has demonstrated a greater influence on health than either genetic factors or access to healthcare services. By implementing routine SDoH screening, pediatric primary care teams will have the opportunity to proactively identify and address SDoH needs, thereby supporting health equity among their patient populations.
The following questions are intended to see if families have specific SDoH needs and to help close care gaps and connect patients and families with resources specific to those needs.
